2025 Care Connections Community Health Worker Scholarship
With generous support from the Mother Cabrini Health Foundation, Care Connections will award up to five stipends of $2,500 to support the recruitment and training of local CHWs.
The goal of the CHW stipend program is to improve access to healthcare services, better manage chronic disease among the target population and build the workforce pipeline.
Eligibility
To be eligible for this scholarship, hospitals or health systems must:
- be located in New York state and/or serving New York state residents;
- participate in HANYS’ Care Connections program; and
- have a mission that aligns with the ethical principles, tenets and teachings of the Roman Catholic faith, according to the Mother Cabrini Health Foundation.
Awards
Award recipients will be selected through a competitive application process based on financial need, impact and feasibility.

- Feb. 3 Application posted
- March 12 Stipend info session
- April 2 Applications due
- April 14 Stipend recipients announced
- April 18 Agreements signed
- May 25 – May 26 CHW completes certification
- May 25 – May 26 Hospital works with PM to develop CHW job description, build scope of work and educate staff on the role of the CHW
- Dec. 18 Interim report and reimbursement request due
- May 18, 2026 Final report due
Participation requirements
Participants must:
- enter into an agreement with HERF to be reimbursed for up to $2,500 for the training and education of a CHW;
- attend a one-hour online presentation on March 12, 2025, including an overview of the stipend program, how to complete a reimbursement request and reporting requirements;
- meet quarterly with a Care Connections project manager;
- develop CHW job description and scope of work and educate hospital staff on the CHW role;
- submit an interim report by Dec. 18;
- submit for reimbursement following payment by the hospital for CHW training/education; and
- submit the final report by May 18, 2026.
