Age-Friendly cross-sector partnership stipend

HANYS is pleased to offer a limited number of cross-sector partnership stipends in 2025 to support the creation or expansion of cross-sector partnerships. Applications are due to actioncommunity@hanys.org by April 4, 2025.

Benefits of participation

• Enhance the quality of life of older adults. By fostering cross-sector collaboration, you directly improve the quality of life of older adults in your community.
• Get ready for the 1115 waiver. New York’s 1115 Medicaid waiver includes a strong emphasis on community partnerships. Use this stipend to start building relationships with your local CBOs so you are ready to be a productive partner in transforming care for the most vulnerable New Yorkers.
• Strengthen future grant applications. These stipend projects can help you build relationships and develop pilot programs that can be leveraged when seeking future grant funding.
• Earn recognition. HANYS shares success stories with our members and policymakers. Your cross-sector partnership could be featured in a future HANYS’ Member Spotlight.

Requirements

To receive a stipend, participants must:

• Be a 501c3 not-for-profit organization;
• enter into a legal agreement with HANYS as a sub-recipient and agree to a detailed work order;
• attend a brief stipend overview meeting with HANYS staff;
• attend bi-monthly progress meetings with HANYS staff;
• identify a community partner and select three outcome measures to track to evaluate the success of the partnership by June 1;
• submit an interim report and payment request form by May 30, using the templates provided;
• participate in a team presentation to the broader HANYS Action Community by Sept. 16;
• participate in a virtual site visit with HANYS by Oct. 15;
• submit a final report and payment request form by Oct. 15, using the templates provided.

Payment

Stipend recipients will be paid according to the following payment schedule:

1. $1,000 to be paid once the recipient completes the following by June 1:
   • Identifies a community partner
   • Develops a project plan using a template provided by HANYS
   • Identifies three outcome measure to evaluate the success of the partnership
2. $2,000 to be paid once the recipient completes the following by August 15:
   • Submits an interim report
3. $2,000 to be paid once the recipient completes the following by October 15:
   • Completes a virtual site visit with HANYS staff
   • Participates in a team presentation highlighting the partnership
   • Submits a final report
