2025 stipends for Age-Friendly Action Community participants
HANYS’ Age-Friendly Health Systems initiative is seeking applications for its 2025 stipend program.
Stipend types
HANYS offers three types of stipends, with varying award amounts, purposes and eligibility requirements.
$20,000 Age-Friendly impact measurement tool stipend
Applications due Nov. 20, 2024
HANYS is offering up to five stipends for sites to build an electronic health record-integrated Age-Friendly impact measurement tool (dashboard). Stipends may be used for information technology staff hours, clinical time and/or other resources necessary to develop the dashboard.
Not-for-profit and public organizations currently enrolled in the HANYS Age-Friendly Health Systems 2025 Action Community cohort are eligible to apply if they:
- have participated in one or more HANYS’ Age-Friendly Health Systems Action Community cohort (in 2021-2024);
- have achieved IHI Level 2 Age-Friendly recognition at the time of application;
- have the clinical and information technology capacity to build an EHR-integrated Age-Friendly impact measurement tool in 2025;
- will commit to tracking six process and three outcome measures (See HANYS’ Age-Friendly Quality Measures Guide and IHI’s Measures Guide for more information); and
- will commit to presenting results and data at a poster session in New York City in late 2025.
$10,000 Age-Friendly flex stipend

HANYS is offering up to nine flex stipends sites can use to jump-start their Age-Friendly work. These funds may be used for patient engagement for co-design and/or for staff hours spent on the organization’s Age-Friendly program.
Not-for-profit and public organizations currently enrolled in the HANYS Age-Friendly Health Systems 2025 Action Community cohort can apply for this stipend. Preference will be given to organizations that have not previously received a stipend under the Age-Friendly Action Community initiative.
Age-Friendly cross-sector partnership stipend

HANYS anticipates offering a limited number of stipends to support the creation or expansion of cross-sector partnerships. Details will be released soon.
Selection and award process
Applications should be submitted to actioncommunity@hanys.org. All applicants will be notified of the outcome of their submissions via email by spring 2025.
Recipients will be selected based on demonstrated financial need, project feasibility (including a draft budget), potential impact and sustainability. More than one care setting affiliated with a health system may apply for and receive funding.
Recipients will be expected to sign an agreement outlining the use of funds, reporting requirements and payment schedule. For more information on recipient requirements, please see the appropriate stipend application.
