Care Connections focuses our work on three regions within New York state. These regions are:

• New York City;
• North Country; and
• Western New York

If you are a hospital in one of these regions looking to better prevent and manage chronic disease, support hospital-community partnerships and build a pipeline of community health workers, please reach out to us!
