Care Connections is a HANYS learning collaborative focused on expanding access to care to better prevent and manage chronic disease. Care Connections will support hospital-community partnerships and build a pipeline of community health workers.

Hospitals that serve patients who experience a high burden of chronic disease and face access barriers will be invited to join the collaborative. Connecting these populations to services will improve care, outcomes and quality of life for the most vulnerable New Yorkers. Hospital teams will receive education, data and technical assistance from a dedicated team of project managers and our expert faculty.

In collaboration with our members, community-based organizations and other institutions and leaders, HANYS is committed to systematically and intentionally closing economic, racial and health gaps in every New York community. HANYS believes that closing these gaps will enable every individual to achieve optimal health through the delivery of equitable health services.
