Best practices in action: Comprehensive dementia care

More than 400,000 New Yorkers live with Alzheimer’s disease and related dementias. To help providers across the continuum of care better meet the needs of these patients and their families, HANYS and the Alzheimer’s Association developed a three-part webinar series, Modernizing dementia: The new dementia care landscape, featuring case studies from New York hospitals that address best practices and new developments in treatment.

A dementia diagnosis is just one part of a person’s medical picture. Patients often have other physical and emotional needs that can impact care. This session will share key elements of comprehensive, evidence-based dementia care models.
This webinar series is part of HANYS’ Commitment to Excellence program, designed to support members in meeting their strategic goals related to quality, patient safety, efficiency, workforce and other key performance indicators.
