How Do You Measure Successful Behavioral Health Integration?

This webinar, cosponsored with our partners across the continuum, will discuss the integration of behavioral health and primary care as it relates to the waiver and the Delivery System Reform Incentive Payment (DSRIP) program. Health Management Associates (HMA) will provide insight on measures providers can focus on to achieve successful integration. HMA will also discuss strategies to integrate behavioral health services in nursing home settings. The webinar will be led by Nancy Jaeckels, Principal at HMA. Nancy is veteran coach and consultant for performance improvement and has many years or experience in helping organizations improve processes and outcomes.
The webinar will focus on how to measure successful integration with an emphasis on these three Domain 3 DSRIP projects:
3.A.I Integration of primary care services and behavioral health
3.A.IIIImplementation of evidence based medication adherence program (MAP) in community based sites for behavioral health medication compliance
3.A.V Behavioral Interventions Paradigm in Nursing Homes (BIPNH)
