Diabetes Collaborative General Diabetes Education
January through May, 2011

As part of HANYS’ Diabetes Campaign Collaborative, HANYS holds monthly collaborative Webconferences on key diabetes care and management practices. The January through May, 2011 sessions cover a number of cutting edge issues around Patient Centered Medical Homes and innovative delivery systems. Therefore, we are opening these specific sessions up to all members.
HANYS' Diabetes Campaign Collaborative promotes diabetes care excellence, chronic care management, and patient-centered medical homes. These Webconferences will focus on the coordination of diabetes care from different points of view within the healthcare system; as well as different ways of improving diabetes outcomes in your patient population.
